• Required  15 min. phone consultation

• Mandatory online paperwork which must be done 24 hours before 1st session or session needs rescheduled.. (details will be given during phone consultation).
   

• Proof of guardianship for a minor by both parents.

What To Expect

​

The first counseling session will consist of working on diagnosis, information gathering, and additional paperwork when the client comes in. Original intake paperwork must be completed 24 hours before your 1st session or the session will be rescheduled/cancelled.  Additional appointments will be scheduled after the 1st session or it may be determined that a referral is needed for further services.
